Thank you for your interest in the BACR Schools Expanded Learning Program—we’re so glad you’re here! This program is offered by Bay Area Community Resources (BACR), a nonprofit organization that has supported children and families across the Bay Area since 1976.

For over 30 years, BACR has provided safe, caring, and engaging expanded learning (EXL) programs where students can get homework help, explore new interests, stay active, and build positive relationships. We believe every child deserves access to high-quality, enriching opportunities where they have fun while learning and feel welcome and safe. Our programs are funded by State and local grants, and we collaborate with the school communities we serve to make them affordable for all families.

RELEASE OF LIABILITY, WAIVER AND INDEMNIFICATION AGREEMENT

I, the undersigned, understand the nature of the Expanded Learning Program and that my child’s participation is voluntary. I acknowledge that Bay Area Community Resources (BACR) assumes no responsibility or liability for injuries or illnesses that may occur during program participation. I agree to indemnify, hold harmless, and release Bay Area Community Resources (BACR), the School District, and their officers, directors, employees, agents, and volunteers from any and all liability, claims, or losses arising from or related to my child’s participation in the Expanded Learning Program, including claims for injury, illness, death, loss, or damage.I understand that participation in program activities involves inherent risks, including the risk of injury or illness (such as communicable diseases, including MRSA, influenza, and COVID-19). While safety rules, equipment, and supervision are in place to reduce these risks, I acknowledge that serious injury, illness, permanent disability, or death may still occur.With full knowledge of these risks, I voluntarily assume all risks of injury, illness, damage, or loss to person or property that may occur while my child is participating in, attending, preparing for, or traveling to and from the Expanded Learning Program.


RELEASE / PICKUP POLICY

All students must be picked up by the end of the scheduled program, Monday through Friday (some program times may vary). The BACR Expanded Learning staff will attempt to reach the people listed on the emergency contact sheet after the scheduled program ends. If we cannot reach anyone on the contact list, ongoing late pick-ups may result in removal from the program. Please keep in mind that our staff also have families and commutes. The BACR ExLP staff understand emergencies occur. If a family knows they will be late due to an emergency, it is up to the family to contact the ExLP Coordinator before the scheduled end of the program. The BACR ExLP staff are required by law to report to Child Protective Services or Law Enforcement. All students still at school sites past 7pm without any caregiver communication will result in Child Protective Services or Law Enforcement. The BACR ExLP staff are required by law to report to Child Protective Services or Law Enforcement.


WAIVER OF PICKUP POLICY

If I arrive later than the dismissal time or am unable to pick up my child at the end of the Expanded Learning Program, I give the staff permission to release my child from the BACR ExLP Program without supervision. This only applies to students in 4th grade or higher.


AUTHORIZATION FOR RELEASE OF CONFIDENTIAL INFORMATION

I grant authorization for the release of confidential information to the school district and/or medical staff as it pertains to my student(s) progress and behavior in the After School Program.


PHOTO/VIDEO RELEASE OPT OUT

During your child’s attendance in the BACR Expanded Learning Programs students may participate in an activity that is being photographed or videotaped; these photographs/video recordings may be used for promotional and educational purposes only.

I authorize the BACR, or any third party it has approved to photograph or videotape my child during the After School Program activities and to edit or use any photographs or recordings at the sole discretion of BACR. I understand that my child and I shall have no legal right or interest arising from the recording, including economic interest. I also agree to release and hold harmless BACR, or any third party it has approved from and against all claims, demands, damages, and liabilities arising out of or use of the recording.



Data Collection

I authorize (give permission) to BACR Expanded Learning staff to collect information via online or written surveys, questionnaires, interviews, and focus groups from the minor child listed on this application. Any and all information received will be kept strictly confidential. Data gathered through these means will be summarized in the aggregate and will exclude all references to any individual responses. The aggregated results of these analyses may be shared with BACR staff, funders, and other community stakeholders for evidence of program effectiveness and impact on our program participants. This release may be revoked at any time by contact in BACR in writing.
